Complete the wellness program by June 30, 2026
To be eligible for a rebate in November of 2026, employees (and their spouses/FIPs,
if applicable) will need to complete 8 points total - 2 required activities and 6
additional. There are additional requirements to be eligible for the rebate. Review
our wellness webpage for all details.
Points must be recorded in Personify Health's platform on or before June 30, 2026
There is no runout period. Points will not be accepted after June 30, 2026. Be sure
to complete the program and record all points in the portal on or before the last
day of the plan year for credit.
Required points
- [Required] Health risk check survey
- [Required] Biometric screening
- Last day to order a mail order test kit is May 31, 2026
- Earn 6 additional points of your choosing
