Wellness Plan Rebate

The wellness rebate will be paid out to employees and their spouses/FIPs who earned the incentive with the paycheck on Friday, November 14, 2025. 

Confirm Your Rebate
To confirm you have completed the program, log into Personify Health and click on "Rewards" under the "Home" tab at the top of the page. Then click on "Earnings History" under the gray box. Navigate to the plan year from October 1, 2024 - June 30, 2025. If you see at least 8 points (any 8 points) you have completed the program.
