HR Rolls Out New In-Grade Form for Supervisors and HRCs

1. What if I have requests already in process using the old template?
- Personnel will complete any forms that are still in process.
2. Using the new template
- It is required to use the new template beginning Monday December 8, 2025.
- Personnel will reject any new entries using the old template.
3. What are the biggest changes to the form?
- Reduced to one page (Form Guidance link available on the form template & compensation request website).
- Email notifications are improved with more relevant information and guidance.
- The employee will not be notified of a form approval/denial. It is the supervisor’s responsibility to meet and discuss this with the employee.
- New Effective Date Process (See FAQ section of the Form Guidance)
- Supervisor Attestation Check Boxes - No attachments required.
- Return for Revision- new guidance on correcting the form while in process. See FAQ section of the Form Guidance.
- UA Compensation is no longer involved in the approval process and will only receive email notifications for all approvals and denials. It is the supervisor and department’s responsibility to audit the form information and ensure it is correct. UA Compensation will run periodic audits.
- Once Personnel processes the form, it will be copied and uploaded to OnBase via an automated workflow. Denied forms will be deleted when they are uploaded to OnBase via an automated workflow and the original forms will be archived in NextGen.
