Software Request process form updates

UA is streamlining the Software Request process to simplify the form while upgrading how we review vendor risk and accessibility. If you submit software purchase requests for your department, please review the changes below before your next submission.

What is Changing:

  • You will no longer need to check the Software Register before submitting a request.
  • Software that has already been reviewed this calendar year will be processed more quickly. 
  • Due to new ADA accessibility requirements, a Voluntary Product Accessibility Template (VPAT) is now required for every request. 
  • If the software is being purchased from a foreign company, you will need to attach the Restricted Party Screening Form and either IRS Form W-8BEN-E or Form W-9 directly on the request. 
  • We have updated our guidance on why these forms are required and where to find them.
  • The "Journal Voucher (JV)" purchase option from the Desired Purchase Method section has been removed.,
  • We have reorganized the "Before You Submit" checklist into four sections for clarity. This will help you gather everything you need before starting the form.
