Complete and then fax or mail the following form: "Report of Injury or Loss" to the System Office of Risk Services in Fairbanks

Immediate notification is required by phone (907) 450-8150 if serious injury or death has occurred. If information is not complete, DO NOT DELAY -- many details can be obtained later.
