Compliance Assistance and Assurance Program Questionnaire

Universities across the nation have been under heightened scrutiny by regulators of environmental health and safety laws. The Environmental Protection Agency (EPA) has notified university presidents and top administrators that the institutions under their leadership are going to be held to the same standards of compliance as industry. Moreover, they stress, that educational institutions have a critical role to play in educating future leaders on working in a safe and regulatory compliant manner. To date, hundreds of universities across the nation have been subject to inspection and penalty, with individual fines sometimes exceeding a million dollars.

As sensational as the regulatory fines have been, UA is more concerned with assuring that faculty, students and staff have a safe and professional working environment. Legally, and under university policy, principle investigators are responsible for compliance management in their areas. Professional staff from risk services departments at each of the main campuses (Anchorage, Fairbanks, and Juneau) are available to assist you in identifying compliance requirements, develop responsive programs, and helping you to implement and manage those programs.

The following questionaire is designed to help the risk service professional understand your current compliance status and help identify where you may need further assistance. If you are unsure  how to answer a question,  the question is unclear, or you would like additional information please check the box  “More Information” and you will be contacted.

"Administrative Procedures" is called Section 1 in our survey and needs to be completed by all responders. Section 1 will appear after the following exclusionary questions have been answered and submitted.

Section 2. Chemical Hygiene Plan (CHP): Is the area under your operational control a laboratory and are chemicals ever used or stored in this area?



Section 3. Personal Protective Equipment (PPE): Are there any recognized hazards in the area under your operational control? Examples of recognized hazards include, but are not limited to, chemical exposures, radiological exposures, sharp objects which may cut or puncture the skin, excessive noise, heavy objects which may fall on a person, flying debris which may be inhaled or may strike the eyes, laser energy or other non-ionizing radiation, or any other hazard which may cause injury, illness, or impairment by inhalation, absorption, ingestion, injection or mechanical action.



Section 4. Biological Hazards (Laboratory): Is the area under your control a laboratory and are biologically hazardous materials ever used in this area?



Section 5. Biological Hazards (Non-Laboratory) Bloodborne Pathogen Exposure Control: Do individuals working under your supervision have the potential to be exposed to blood, blood products, bodily fluids, or any other biological materials?



Section 6. Laser Safety: Do the operations under your supervisory control have or use lasers?



Section 7. Radioactive Material: Do the operations under your supervisory control have or use radioactive material, including open or sealed sources or in machinery or equipment? Examples include x-ray machines, electron microscopes, various types of non-ionizing radiation sources like ultraviolet, magnetic, and RF emitters.



Section 8. Hazardous Materials and Waste: Does the lab under your operational control have or use hazardous chemicals and/or generate hazardous waste?



Section 9. General and Mechanical Safety: Does the lab under your control use mechanical, electrical, or other specialty equipment?



Section 10. Field Research: Do you conduct research at locations off UA campuses (outside the geographical boundaries of the University's campuses, identified as a field research station, or located at another University operated facility)?
