UA Student Accident Insurance
General Coverage Information
Student accident insurance coverage is available, upon request, to University of Alaska students through the System Office of Risk Services, in Fairbanks.
The specific criteria for coverage for this insurance is that the event or program be:
University sponsored; and
University supervised (by UA staff member).
This insurance covers accidental injuries occurring during the insured period but does NOT cover personal health or sickness. The coverage is excess to other insurance available to the student. In other words, this insurance policy will cover expenses after other policies an injured student may qualify for under the student's personal insurance, spouse's insurance, or parent's insurance have been exhausted.
Premiums vary depending upon the type of activity in which the student is participating as described on the application form by the student or department.
To request coverage, please complete the Student Accident Insurance Application and forward to the System Office of Risk Services, in Fairbanks. Additional Class/Group Enrollment forms maybe copied for situations with multiple enrollees.
Please also print out the Student Accident Insurance Brochure which describes the general terms and conditions of the policy and contact information. A brochure needs to be given to or made available to all students who have applied for coverage.
The Student Accident Insurance Election/Rejection Form can be used by departments when they are offering students the choice to enroll in student accident insurance. This form is for department use only and does not need to be submitted to Risk Services with the application.
The form must be completed and received in the System Office of Risk Services, in Fairbanks prior to the date for which coverage is to be effective.
COVERAGE DOES NOT GO INTO EFFECT UNLESS COMPLETED APPLICATION FORMS AND PREMIUM PAYMENT HAVE BEEN RECEIVED IN THE SYSTEM OFFICE OF RISK SERVICES.
The forms maybe faxed to (907) 450-8151. Our fax machine will date and time stamp incoming insurance applications.
Premium payments are to be arranged by departments enrolling multiple students. Premiums will be debited from departments via journal voucher at the end of each month. Students applying individually must deliver their premium payment along with the completed application form to the System Office of Risk Services.
