This form should be completed and submitted online. You can also download a PDF file to submit, but submitting online is the preferred method. There may be processing delays if this application is not submitted electronically.
Step 1:
To receive consideration for need-based scholarships, you must submit a completed Free Application for Federal Student Aid (FAFSA) by February 15 prior to the semester you plan to attend. FAFSA data is used to determine scholarship eligibility including financial need, number of dependents other than spouse, etc. These items are selection criteria for specific scholarships and an important part of your application.
Step 2:
Go to UAOnline to apply for admission to the UA Campus you plan to attend.
Step 3:
Apply for scholarships on UAOnline to be automatically considered for all scholarships. Once logged in, choose Scholarships located in the Student Services menu; answer the required scholarship questions. The UAOnline portion of the scholarship application is required and you will not be considered for any scholarships if you have not completed it.

Return to this page and complete the form below for scholarships that require additional information not accessible via UAOnline.
Deadline for most scholarships are February 15, 2013 for Fall 2013-Spring 2014 scholarships. If received after this date we will hold on to the application until July 15, 2013, to be used for any additional scholarships that open up. Any applications received after the July date will be disqualified. Submitting an incomplete application or failing to adhere to the requirements will result in disqualification.
NOTE: Information provided on this form will be used by the UA Foundation for University related business only. This form resides on a secure server. Our secure server software (SSL) is the industry standard for secure transactions. This software encrypts all entered information so that it cannot be read as it travels through the Internet.
IMPORTANT NOTE: You will not be considered for scholarships unless you have also applied for admission to the appropriate undergraduate or graduate degree program at the UA Campus you plan to attend. All selection criteria not requested on this form or through UAOnline (see step 1 above) will be obtained from your UA student record (e.g. academic major, class standing, etc.). You must be sure your UA student record is as complete and accurate as possible. A personal essay is a required part of the application (UAOnline; step 1). It is recommended that you compose your essay in a word processing program such as Microsoft Word, then copy and paste it into the essay box.
