FY15 Benefits Open Enrollment

April 15 - May 16, 2014

Welcome to Open Enrollment 2014. Open enrollment is the employee's annual opportunity to make changes to certain benefits for the new plan year, effective July 1, 2014.

All information is here on-line and no packets will be mailed, so be sure to read the material.  If there is anything you don't understand, please ask questions!

During open enrollment, you can:

Add or opt out of health plan coverage

Choose your new plan option

Enroll eligible family members in your health plan

Add, drop, or change the level of your life insurance or accidental death and dismemberment (AD&D) coverage

Enroll or re-enroll in a flexible spending account to pay for day care or health expenses with pre-tax dollars

Forms You Will Need

FY15 UA Choice Enrollment Form (health plan rates located at the end of the document)

FY15 CDHP / HSA Enrollment Form (also use this if you want to just re-enroll in the HSA)

Supplemental Benefit Election Form (Includes Medical and Dependent Care Flexible Spending Accounts and Supplemental Life Insurance - rates on page 2)

Health Plan Opt Out Form (if you're currently opted out and want that to continue, you don't need to turn in another opt out form)

Reference Materials

The Affordable Care Act requires that we provide summaries of benefits and coverage for each plan we offer, as well as a a Glossary of Terms for terms used in insurance and health care discussions.  Each summary provides information about what the plan covers, with examples of common health scenarios (treating diabetes, and having a baby) so you can compare what you'd pay under each plan in a consistent format.
