All UA Choice plans provide coverage for preventive, restorative and prosthetic dental services, described below.
Claims/Coverage Questions
The Blue Cross customer service number is (800) 364-2982 and the TDD for the hearing impaired is (800) 842-5347. Claims information and other health resources are available on the Blue Cross site. Members create their own user ID number and password when they register. To register, however, requires a six-digit PIN number. If you do not have one, call (800) 722-9780 for help.
Major Categories of Dental Work
Preventive work: includes oral exams and cleanings, X-rays for diagnosis, and fluoride treatment and sealants for dependents ages 15 and younger
Restorative work: includes temporary and permanent fillings, extractions, periodontal exams, root canal therapy and the repair and recementing of crowns, inlays, bridgework and dentures
Prosthetic work: includes inlays, onlays, gold fillings and crowns, the initial installation of dentures, replacement inlays, onlays, crowns and fixed bridgework under certain conditions
