Student Enrollment Verification
Continuing Full-time Students on Health Coverage
Full-time students may continue to be covered under the university's health care plan after they turn 19. To be eligible, the student must be:
- unmarried;
- under 24 years old;
- primarily dependent on the employee for support; and
- attending an accredited educational institution.
Premera Blue Cross mails out an enrollment verification form twice a year to document a student's full-time status. You may obtain documentation several different ways.
If your student does not attend the University of Alaska:
- ask the school's registrar to fill out the form; or
- alternatively, you may download an enrollment certificate through the National Clearinghouse Web site (off site) for a fee. A guide (pdf) offers step-by-step directions for using the site. The enrollment certificate and the Blue Cross Student Enrollment Verification/Dependent Health Care Eligibility Form must be mailed to the Premera address on the form.
If your student attends the University of Alaska:
- You are encouraged to download an enrollment certificate through UAOnline at no charge.
- Directions are below.
Downloading a certificate through UAOnline:
- Go to UAOnline.
- Have your student log in to the secured area.
- Choose Student Services, Financial Aid and Account Information.
- Click on Enrollment Verification for the free service offered by the National Student Clearinghouse.
- Click on the icon for the school your child attends (UAA, UAF, or UAS).
- Go to the “Verification Clearinghouse Web site.”
- Choose “current enrollment” and “obtain an enrollment certificate.”
- Print it out. The enrollment certificate and the Blue Cross Student Enrollment Verification/Dependent Health Care Eligibility Form (pdf), must be mailed to the Premera address on the form.
