Using the $750 Preventive Benefit

The UA Choice Health Plan includes a $750 preventive benefit for routine and preventive services, including physical exams and laboratory and x-ray services performed in connection with these exams. Benefits are provided at 100 percent of the allowable charge and are not subject to the deductible. The preventive benefit is available to you in addition to traditional diagnostic care. It is important that you understand how this benefit works so that you can use it effectively.
